Example: “My sleep and anxiety have changed in the last 2 months and it’s affecting work.”
2) Pick your top 3 symptoms
- Severity range (1–10)
- Frequency pattern (once / few times / multiple / constant)
- 1–2 notes: what was happening, what helped, what didn’t
3) List what you’re currently taking
- Medications and supplements (names are enough)
4) Decide what you want from this visit
- Symptom relief plan
- Rule-out guidance
- Treatment options discussion
- Follow-up plan

- “What are the most likely explanations for my symptoms right now?”
- “What should we try first, and how long before we reassess?”
