Before your appointment (10 minutes)

1) Write your “why now” (one sentence)

Example: “My sleep and anxiety have changed in the last 2 months and it’s affecting work.”

2) Pick your top 3 symptoms

  • Severity range (1–10)
  • Frequency pattern (once / few times / multiple / constant)
  • 1–2 notes: what was happening, what helped, what didn’t

3) List what you’re currently taking

  • Medications and supplements (names are enough)

4) Decide what you want from this visit

  • Symptom relief plan
  • Rule-out guidance
  • Treatment options discussion
  • Follow-up plan

Questions to bring (copy/paste)

  • “What are the most likely explanations for my symptoms right now?”
  • “What should we try first, and how long before we reassess?”
  • “What changes would mean I should message you sooner?”
  • “If we consider treatments, what are the pros/cons for me?”
